Discussion Questions: After reading chapter one, and the article “Where We’ve Been: Punitive Culture, Blame-free Culture, and Just Culture”, what do you think? Is the system to blame, or should we fear that if we begin to blame the system then we err by not “punishing” the “person(s)” committing the wrong? Will the creation of just cultures “fix” the problem and/or do you have any experience with a just culture?
