Our discussions this week will focus on reviewing Domain 1: Data Content, Structure, and Standards, which corresponds to Course Objective 2.

Once you review the relevant material, discuss the following.

1. Discuss health information documentation by focusing on the format of the health records at your facility (paper, electronic, hybrid), documentation requirements, the legal health record, and the master patient index.
2. Discuss the flow of information from the moment the patient is registered, admitted to a certain unit, and discharged. When does the health information department start working with the health information? What are some of the documentation analysis or other processes that are performed in the HIM department?
3. Share any practicum updates, comments, or concerns that do not fit the two topics above.
