Part of the responsibility of the Health Information Management professional is to understand and know where data is located in patient records and in what type of patient records. Some documentation may be found in a physician practice record but not in the acute care hospital record. Knowing where to locate information in the patient record is also critical for coders to be able to code and support the clinical diagnoses in the record.
